ถ้าผู้ขนส่งเรียกเอาใบกำกับของ ผู้ส่งต้องทำให้ใบกำกับของนั้นต้องแสดงรายการต่อไปนี้ คือ(๑) สภาพและน้ำหนัก หรือขนาดแห่งของที่ส่ง กับสภาพ จำนวน และเครื่องหมายแห่งหีบห่อ(๒) ตำบลที่กำหนดให้ส่ง(๓) ชื่อหรือยี่ห้อ และสำนักของผู้รับตราส่ง(๔) ตำบลและวันที่ออกใบกำกับของนั้นอนึ่ง ใบกำกับของนั้นต้องลงลายมือชื่อผู้ส่งเป็นสำคัญ

English translation
Where the carrier requires a waybill, the consignor must provide one. The waybill must state:
(1) the nature and the weight or measurement of the goods sent, together with the nature, number and marks of the packages;
(2) the place of destination;
(3) the name or trade name and the address of the consignee;
(4) the place and date of the making of the waybill.The waybill must be signed by the consignor.
