Two ZIP downloads are supplied: an editable Word pack and a matching PDF reference pack. Unzip the files and read the included guide. Each document puts English instructions first, then Thai instructions. The bilingual instrument begins on a separate page, with signature blocks and separate attachments. Logos appear only on instruction pages.

For blank lines, follow the English and Thai label and leave signatures until signing.
