ถ้าผู้ส่งเรียกเอาใบตราส่ง ผู้ขนส่งก็ต้องทำให้ ใบตราส่งนั้นต้องแสดงรายการต่อไปนี้ คือ (๑) รายการดังกล่าวไว้ในมาตรา ๖๑๒ อนุมาตรา ๑, ๒ และ ๓ (๒) ชื่อหรือยี่ห้อของผู้ส่ง (๓) จำนวนค่าระวางพาหนะ (๔) ตำบลและวันที่ออกใบตราส่ง อนึ่งใบตราส่งนั้นต้องลงลายมือชื่อผู้ขนส่งเป็นสำคัญ

English translation
Where the consignor requires a bill of lading, the carrier must provide one. The bill of lading must state:
(1) the particulars set out in Section 612 (1), (2) and (3);
(2) the name or trade name of the consignor;
(3) the amount of the freight;
(4) the place and date of the making of the bill of lading.The bill of lading must be signed by the carrier.
