หนังสือตั้งผู้รับฉันทะนั้น ให้ลงวันและลงลายมือชื่อผู้ถือหุ้นและให้มีรายการดังต่อไปนี้ คือ(๑) จำนวนหุ้นซึ่งผู้มอบฉันทะนั้นถืออยู่(๒) ชื่อผู้รับฉันทะ(๓) ตั้งผู้รับฉันทะนั้นเพื่อการประชุมครั้งคราวใด หรือตั้งไว้ชั่วระยะเวลาเพียงใด

The instrument appointing a proxy shall be dated and signed by the shareholder and shall contain the following particulars: (1) The number of shares held by the shareholder (2) The name of the proxy (3) The meeting or meetings or the period for which the proxy is appointed.
